Prior to your Appointment

Please attend your first consultation 10 minutes prior to the appointment time to allow completion if initial patient information paperwork unless completed online prior

You may be required to bring your referral paperwork with you if it has not been forwarded to the practice before-hand

Attending with a list of your current medications and relevant medical history is encouraged

You will be asked to provide your medicare / healthfund / veterans affairs or other relevant information to reception

Workers Compensation patients must provide the name of their insurer, a valid claim number, contact information and employment details and required approval from Workcover prior to attending

Payment will be required at the completion of your appointment and a percentage of the consultation cost will be claimable from Medicare in most cases and can be rebated electronically

Please wear clothing that will allow examination of the affected body part
